W-4 – The purpose of Tax Form W-4 is simple ― it is used to withhold the proper amount of federal income tax from your paycheck. Please submit a new W-4 tax form each year, or any time your personal or financial situation changes.

I-9 Employment Eligibility Verification – The Employment Eligibility Verification Form I-9 is a U.S. Citizenship and Immigration Services form. It is used to verify an employee’s identity and to establish that the worker is eligible to accept employment in the United States.

Non-Disclosure/Non-Solicitation – This agreement is the employee’s promise to keep all information confidential (client/customer names, locations, etc) and agrees not to solicit clients or customers, for his or her own benefit or for the benefit of a competitor, after leaving the company.

Direct Deposit Authorization Form – Use this form to receive your paycheck electronically deposited to your bank.

W-9 (Contractors Only) – When Short Stacks pays an independent contractor $600 or more over the course of a tax year, it is required to report these payments to the IRS on an information return called form 1099-MISC. Short Stacks use the name, address and Social Security or tax identification number from form W-9 to complete form 1099-MISC.
